Again, your Processor is the bottleneck, for this game you will not be able to run it because COD 4 requires a 2.4 Ghz processor, you may be able to play but it will be quite laggy. Do yourself a favor and grab a new CPU and that rig should be fine.

Reseat RAM, if you have integrated with a PCI/PCIe card disable integrated, try another port if there is one. Plug in headphones and see if you hear the startup noise. Could also be a faulty PSU/mobo, see if you have another laying around or pull out some parts to draw less power

I use HDAT2 and DFT on hirens boot CD, you can also download programs to run, but I have found those to be more thorough. Pop a CD in the drive to see if the noise is coming from that first.

Have you run any drive scans to ensure its not failing? Sounds like its just vibrating something, or maybe the CD drive was knocked lose? Open up a CD from my computer and listen to it spin.
