I agree with most in that at this point, it's time to take some real measures. Have you tried system restore? Try restoring your machine to a date and time before you got the infection. This will be the best way for a massive fix that will leave most of your precious data intact. Other than that, looks like your best bet is going to be wiping down your hard drive and starting over.
System restore doesn't work that often for removing viruses or anything like that. Usually depends on the infection, but they usually persist past restore points.
I know of one more tool I have sound very useful if you go to bleepingcomputer.com you will find ADWcleaner and junk remover they have helped me out multiple times already .
