I used YUMI to multi-boot both UBCD and Hirens onto the same USB back when I worked in a technician environment, worked great!

Yeah I could dual boot it I guess if I get a default iso.. I customised my hiren iso and added newer diagnostics softwares / graphics etc..

Why couldn't you dual boot your custom?

Should be able to dual-boot it even with a custom one I'd have thought, the limitation isn't on the content of the .iso
