The universal standard in the US is round.

New Hampshire has triangular ones, not sure if any other states do.

Nashua is about the only city that has those. All other cities/towns/hamlets/teepee conglomerations have round ones.

Well oddly as a experiment I lifted the one in my garden which is round an made of concrete an steel (layered) above my head this ting hasn't been moved in over 20 years but I found i easy to pull and lift with no tools, my supasition is this the local department didn't leave it open some one took it for the scrap weight.

The round manhole covers also can not fall into the hole.

Square ones or triangle ones can.
