Can anyone ID this interface?

Dude just buy a new one. That thing is old as dirt. My phone will take better video. I bet a $100 point and shoot would do better.

From what I can tell it is a 4 pin 1394 (AKA firewire) connector. I looked though my old cables and found a 4 pin to 6 pin 1394 cable. I think this is the cable you need and they are still available. They were often used for video on camcorders. My DVD/VHS recorder/player has that connector on the front panel as an input. The 4 pin to 4 pin, 4 pin to 6 pin and 6 pin to 6 pin 1394 cables are still for sale. The 1394 connector on the rear panel of some motherboards are of the 6 pin variety. Hope this helps.
