finding dvi to vga adapter

We have looked everywhere and can't find the adapter that came with the video card. The strange thing about this dvi female slot on the video card is that it accepts a smaller flat piece on the one end and there aren't 4 holes (2 above and 2 below) to accept four pins around that flat piece. I have looked at a lot of sites trying to find this adapter, but may end up buying a new card.

Anybody know where I can get this adapter?

There's only two types, the top one converts a dvi slot to vga, and the bottom one converts a vga to dvi
