PCI Ethernet Shot?

I been having some problems with my internet. At first I thought it was the ethernet wire so I purchased a CAT6, then I thought it was the modem, but now I have a router and and two out of my three nodes work fine. Think it's the network card?
 
test the network card and see what it does. Inside windows go to START>RUN>CMD then at the command prompt type ping 127.0.0.1 this will do a loopback test and tell you if the NIC is good. You should have 100% packets recieved and a very low seek time like 1ms for all 4 packets sent.
 
The internet works some of the time and then it just goes out. Im currently runnin VoIP and when I plug it into the out port on that I never lose connection? This problem is confusing. I have a centralized server for storage and when the computer acts up and I have to dump the line into the VoIP router I cnat access my files. UGH!
