Default multiple keyboards, one computer

Hi, i am working on a project at moment and trying to get some ideas to put together a solution for the following:

multiple keyboards at a booth, each with a small monitor displaying a program which has three inputs (form fields), "age", "name", "message", and an ok button.

this all has to run off the same computer, which is hidden underneath the booth. the program will record what people type in, and store it for later use.

is there any way to achieve this? any hints or leads would be very much appreciated!
