Default Floppy Disk Formatting

Why is it that some of the floppy's I use have to be formatted and some don't and what exactly does that mean anyway?

some come preformatted, some dont

formatting just puts an empty filesystem on the disk, unless the disk is gonna be written to in raw data mode it needs a filesystem on it first
