Default Sometimes Artifacts 0o? ATI 9600 PRO

Hi, I have an offical ATI 9600 PRO (128 MB) video card. It's recently have broken down probably due to drivers crashing since I've used modded drivers and switched back to Offical drivers.

I've sent my video card to ATI for repair. Now that I've got it back, I've no longer used ARTIC COOLING Rev3 as cooling and used its stock fan (but added artic silver) to test and make sure that there are no further problems. However, for some reason sometimes when I restart, it will load into windows with corrupted images (artifacts). It's so corrupted that I cannot see what's happening in windows.

I am not using ATI tool or any modded drivers right now. I am using offical Cataylst 5.8 drivers. Can someone tell me why it's happening and how to fix it?

Underclock it until you see no more artifacts, and check the GPU core and ambient temps.
