Default Reviving a dead SD card?

Hello All, I have a 2GB SD card which i used to use for my phone. A few days ago that went dead. I cant access the card itself either through the phone or through the PC or through a card reader.

Any way I can access this card?

Format it.

My computer uses 1.5A-load, .8A-idle, .65A-standby, .05A-turned off on 125V.
