Graphics card failure?

Is this a graphics card failure or a monitor issue or an overheating problem?

This happened when i was playing GTA IV and the screen went black with a buzzing sound until i force turned my computer off then it had 2 green lines until the login screen. http://m.imgur.com/oQ99O7M

What's your GPU? That looks like artifacting from the VRAM.
