The crystals on the screen refresh at a certain ms rating (8, 16, 12, ect) if they refresh to slow (generally 16 and above) then you will see an afterimage since the crystals do not change fast enough, it is almost like a trail following whatever image is ghosting.

Is that clear?

Basically the crystals go so slow that the newer ones start showing an image before the older ones clear the image.
 
Actually, they don't change at a set time. When a LCD is made, they test how long it takes for the colour to change, and they then take the lowest one and state that as the refresh rate.
 
well they change at a certain rate generally, which is the response time, alot of the 2 ms response times out there are actually 8 ms average or even 16, they just happen to get 2 ms every now and then.
