Default Yet Another Video Card Upgrade

Hey everyone! Five years ago you guys helped me build my first computer, now I'm looking to upgrade, and could use some help choosing a video card.

For years, everything ran great. And then I upgraded my monitor from a 1280x1024 to a 1920x1080 display and now my computer weeps and cries out to God whenever I play any game newer than 2010.

I'm not a gaming fanatic, but I'd like to be able to play Far Cry 4, Witcher 3, and current gen games at least on High settings with decent fps. I'd really like to get something that will make a noticeable difference in FPS without breaking the bank. $300ish is probably as high as I can go.

My specs are:
AMD Phenom II X4 955 Black Edition 3.2GHz
Gigabyte AMD 770 ATX Motherboard
16 GB DDR3 RAM
XFX Radeon HD 5770 1GB

If any of you have any suggestions, I'd love to hear them.
Thanks!

If you can sport 330 then get a gtx 970. If not then get a 280x or 290.

Can I just switch from a Radeon to Nvidia without anything blowing up?
