Default question about mail in rebate

I didn't know where to post this. Please, forgive me.
I'm kind of new to MIR.
I recently bought a cooler master elite 430 which had a $10 mail in rebate.
The MIR form asks me to send the original upc bar.
now on both sides of the box where the case came, it has a sticker with the model number, lot number, EAN, and the UPC
Should I send that entire sticker, or cut out only the UPC part. And should I send both UPC ?

cut out the entire UPC sticker. Might want to make a scan of it as well, for your records.
