Google Docs/online data collection

Which applicatio or website can i use to collect data online through questioneers because google docs has a problem because after i create a form and people respond .. the spreadsheet questions are not arranged inorder plus some show they are blank though filled

There is no application or website you can use. You will have to create your own since you will need a database in order to store the results.
