Move to VoIP - Analog Question

HI,

I am about to move to Cisco VoIP system.

We have a few analog devices that are connected this way :
BT male plug > RJ45 Adapter > RJ45 Port > Network Patch Panel to Voice patch panel > PBX

I have purchased Cisco Analog gateway which have 2 RJ11ports (FX0)

How do I connect from RJ45 patch panel to RJ11 port on gateway?
