Cant connect to my own at home server

So I just switched out my router and modem for a new shiny one, but then realized I have to go through setting up the server ip again. I was looking at the notes from last time I had set it up and have been trying to get it all hooked up, but to no avail. The server is on a seperate machine and last time when I had it setup for me to connect with my main pc the IP was 10.0.0.XX:XXXXX, but I cant seem to find where I located this information. Anyone know?
 
Windows 7 and up, you do it through Network and Sharing Center. Right click your network icon in your icon tray by your clock, and choose Network and Sharing Center (network & Internet Settings on Win10).

On Win10 click "Change adapter options" - on Windows 7 click "Change Adapter settings" on the left side.

Right click your connection -> Properties -> highlight "Internet Protocol Version 4 (TCP/IPv4)" in the box, and then click Properties. There you should be able to change your computer's IP to match what your router is pushing out.
