Really thin lines, like vibrations, moving from the upper left corner to the right lower corner. Starting at 75Hz and it get's worse at 85Hz or higher.
I don't have it with my old card wich am using rightnow, so it's not my monitor.
 
yeah.. sounds like your card is going. I doubt a bios update would help you much. It's too bad.. I just sold my agp 5600 on ebay. Otherwise, I could've hooked you up with a good deal.

I have a 5600, it's an excellent midrange card. They're pretty cheap nowadays, too but I couldn't find one on Newegg, just the 5500 and 5700.
 
Yeah, mine was the 256 mb one also.. except it was by BFG... the Asylum!!
Sold it on ebay for about $76; I think the nice aftermarket heatsink/fan I put on it helped it sell :)
