SATA failing!!!

why does my sata drive always fail??? it just will stop sometimes and say somethign about being unplugged, then i generally restart and its back!!!
any help?

they always seem loose. i dont really know what to do... i was thinking of getting the power/SATA dual cord. does anyone use these? and do they just adapt easy to 4pin molex?

i have always been suspicious that this was it.
