USB Device not recognized?

Can't figure this out. Down in the system tray an icon keeps popping up every couple of seconds saying that there is an USB device that is unknown. The window says, "One of the USB devices attached to this computer has malfunctioned and Windows does not recognize it." Etc. I've tried everything I know to try and figure out which device it is to no avail. It also caused my maching to BSOD. It's also bringing up a "code 45". Any help? Thanks.
 
Unplug all USB devices attached to your PC. Plug them in one by one, waiting 10 seconds between each. When the tray popup pops up, you'll know which one.

The thing is, there is nothing plugged into the USB ports accept my mouse and in device manager there is still an "unknown device" showing up.
 
Are you connecting your mouse through a USB hub at all?
Oh, and can you check that any USB (or other) ports you might have at the front of your system all work.
